COMMUNITY HEATLTH REPRESENTATIVES TRANSPORT POLICY: The CHR Departments will need 2 day notice on local transports, for medical only. Call must be made 2 day in advance before appointment date and time, if canceling, rescheduling, or postponing appointments as matter of courtesy, it is appreciated *CHR office is the last resort for transportation needs* no last minute calls due to daily schedule interruptions take away from client’s needs PLEASE do not ask to be taken to places other than the medical appointment TRANSPORTS EMERGENCIES are a priority an emergency consists of immediate medical care The Ute Tribe CHR program serves the four major Native American Communities of Myton, White rock Ft. FIRE RESTRICTION ORDER: June 13, 2018 fire restriction order for the Uintah and Ouray Reservation will go into effect and continue standing until rescinded. Due to the extremely dry vegetation hot temperatures, daily winds, and recent wildland fire activity, the decision was made to implement these restriction, with summer activities and pow-wow season upon us please be extremely safe when recreating outdoor.
